Questions:

1. A patient comes in late for a medication review. You notice that she has been requesting more of her sumatriptan, but she hasn’t had her pill check for her microgynon.

How do you manage this patient?

2. A mother attends with an 8 month old baby who had been having fevers all day, which are 39.0 and not responding to paracetamol or ibuprofen. She says he has also been very irritable and clingy. On examination, you note that the anterior fontanelle is bulging, and a non-blanching maculopapular rash is starting on the torso. You are concerned about meningitis and call an emergency ambulance for the patient.

What else should you do while waiting for the ambulance?

3. A 24-year-old girl presents with a cough and tightness to her chest. She has been using only her Ventolin inhaler, and today she needed to use more of it- saying she's had 12 puffs. On examination, her peak flow is reduced by 20%, you can hear wheeze throughout her chest, and crackles at the base of her left lung. Her observations are as follows: oxygen saturations 96% on air, respiratory rate 18, heart rate 80, temp of 38.0 and BP of 112/78.

Would you refer this patient to A&E or manage her in the community? If she is for community management, what would you prescribe for her?

4. A 65-year-old gentleman presents for his BP check a month after starting treatment for high blood pressure. He takes amlodipine 5mg a day, but his recent BP readings at home have been showing to have persisted at 168/100 from 179/109. He denies any headaches or side effects.
