Rail | QF405 Record of Annual Suitability Review

The purpose of this review is to determine that a Primary Sponsored Worker remains suitable for work on Railway Infrastructures. This will be completed though a discussion with the individual’s line manager and review of records held on Coyle Personnel Systems.

Record of Review

Does the Worker have current proof of Right to Work in the UK?

Has the worker completed a Rail Medical Questionnaire in the past 12 months?

If any emerging or worsening health conditions were identified have these received the appropriate action? (Refer to HSQE Manager or Occupational Health Support where necessary)

Is there any record of poor safety behaviours by the Worker in the past 12 months?

Has the Worker been involved in any Accident, Incident or Near Miss in the past 12 months?

Has there been any incidence of misconduct by the Worker in the past 12 months?

Were there any concerns expressed by the Worker’s Line Manager on the Worker’s performance/reliability during the past 12 months?

Were there any matters involving the Worker that resulted in a disciplinary outcome in the past 12 months?
